ATHLETIC TRAINING ROOM GUIDELINES

• This is a medical facility - Treat it
like one
• The training room is no excuse to be late for practice - Make the time
• No shower, no treatment
• No food in the training room• No muddy shoes in the training room. Shoes that are worn for softball practice or games are not to be worn in the CAC
• No shoes on the tables
• Training room hours: 1:00 pm to
6:00 pm M-F

Pre-Participation Physical Exams:

• Each Athlete must undergo a Pre-Participation Physical Exam (PPE's) and receive clearance from the team physician prior to each season before the athlete may begin workouts. The Athletic Training Staff will schedule PPE's in conjunction with the team physician and the BYUH Health Center.
• Any Athlete that is trying out for a team must have a physical form (link) signed by their own physician along with proof of insurance prior to the two-day tryout. This needs to be turned into the Athletic Training Room. All injuries during tryouts are the responsibility of the athlete trying out.

Injuries:

Report all injuries to the training room & your coach, no matter how small. We may be able to help.
You are responsible for your injuries. If treatment does not seem to be helping, let us know. Don't just stop coming.
THE TRAINING ROOM IS NO EXCUSE TO BE LATE FOR PRACTICE. Make the time to come to the training room for treatment and rehabilitation. If you have a late class, let your coach know ahead of time so arrangements can be made.
• The training room will schedule all referrals if you do need to see the team physician or anyone at the Health Center.
• All eye problems and concerns need to be seen by the athletic trainers to be scheduled with a preferred physician.

Physician visits:

Physician referrals are to be made by the Athletic Training Room for all athletic related injuries. If a physician appointment is made and you miss the appointment without contacting either the training room or the physician's office (BYUH Health Center included), then you will responsible for paying for any re-scheduling fees.
• If you go to see the doctor without coordinating with the Athletic Trainers (not student trainers) then you will be responsible for all of the billing.
• You are responsible for paying for non-athletic-related physician's visits. These include, but are not limited to, illnesses or conditions not related to actual participation in or training for your sport.

Insurance:

• You must provide the training room with original insurance forms (link) signed by your parents & copies of insurance cards.
• It is the athlete's responsibility to bring any billings or statements that are received to the training room. If they are brought in after 30 days they become your own responsibility.
• The school will pick up what personal insurance does not.
• If your personal insurance does not cover regular office visits in Hawaii, you will need to either: 1) sign up for school insurance (DMBA) at the Human Resource Office or 2) provide verifiable proof that the policy has been amended for that coverage in Hawaii.

Equipment Checkout:

• If you lose it, you pay for it.
• Turn it in at the end of the season or injury.
• If it wears out then bring it in for repair or replacement.

Medicine & Supplements:

• We need to know what you have taken or are taking on a regular basis for drug testing and for medication interactions.
• Drug testing will be done on a random basis throughout the school year, so you will need to keep track of these items for yourself.
