Checklist for requesting use of BYUH facilities

 

Step one:

Acquire the following information:

 

·        Name of the entity requesting the use of the facility.

 

·        Type of event – personal, educational, church event, etc.

 

·        The audience (public, family, church, etc.)

 

·        Number of people expected to attend the event.

 

·        Date(s) & time(s) of the event.

 

Step two:

Determine the following:

 

·        Will additional security be needed? (This is determined by the size of the event.)

 

·        Are there any special needs?

 

·        Request a certificate of insurance (COI).

 

The certificate must include the following language, “certificate holder is named          

     as additional insured for…”, then have them list the type of activity the insurance

     is covering with the date and time.

► The COI then goes to Risk Management for their records.

 

Step three:

Contact C.I.T.O. (Center for Instructional Technology and Outreach) Diedra at 675-3784

 

·        C.I.T.O. will take your request to the calendaring/scheduling committee for approval.

 

·        On occasion, the request may be sent to President’s Council for approval. This is on a case by case basis.

 

·        It will be determined in the meeting whether or not other waiver forms need to be signed.

 

Step four:

We monitor the activity and inform Security of the event.  

·        Complete an evaluation form regarding the activity and turn into Diedra.
